IKÄPIHA – harmonisation of the grounds for granting long-term elderly care services in counties

Abstract

Equality is a key value of the Finnish social and healthcare service system with legal grounds. However, there is a regional
variance in the use of elderly care services due to, at last partially, other reasons than the age structure and morbidity.
The purpose of the report is to examine the current structure of elderly long-term services and to estimate effect of
harmonisation of the grounds for granting services on the coverage of the services and the costs in counties. The report
describes the situation on harmonisation of the services in some counties, in which already currently exists one organising
body. Additionally, the report estimates potential calculated effects of harmonisation.
Report shows that harmonisation would have significant effect on the coverage as well as the costs of the elderly’s log-term
services. If harmonisation was implemented according to the example service structure presented in the report, this would
lead to half a billion-euro cost savings annually on a national level combined. However, this calculation is merely theoretical,
since it is based on modelled costs. Further, extensive development work to lighten the service structure would be needed to
achieve savings.
Harmonisation would mean an even more unitary service structures and a more equal entry to services than currently,
which in itself is a valuable end result. It was observed, that harmonisation was already partially adopted in some counties,
which had harmonised their services. Harmonisation is one factor in organising the elderly population’s services equally
and financially sustainable, but the service needs and costs need to be affected, in the future, simultaneously in various
different ways.
